Transition to Independence

Spring Healthcare Limited is devoted to fulfilling, person-centred approach in all aspects of a young person’s life. We do this by providing a programme to our young people called Transition To Independence. We’re a semi-independent living service, looking after young people and care leavers aged 16 – 18 years old (can be extended past 18 if required). Our Transition to Independence aims to support local authorities. In their duty to safeguard and promote the welfare of young people looked after and leaving care. This allows our young people we support to benefit from a sufficient accommodation. Therefore meeting the needs of our looked after young people. This allows Spring Healthcare Limited to fulfill, our ethos “Committing to provide a positive impact in young people’s lives”

Our Transition to Independence values are a vital factor in improving placement stability. Which in turn is a critical success factor in relation to better outcomes. As we aim to work with the young person and their families/carers as well as all other relevant professionals to find a way forward.

They are supervised by our friendly, well-trained health care workers. On leaving the programme, participants will be equipped with practical life skills. That will enable them to make the most of their lives and the communities around them. This is available across the Northwest, our semi-independent houses are conveniently located close to local shopping centres, amenities and public transport.

Support needs will cover a wide range of issues and include: isolation; abused or exploited children; mental health and young children where the parent has suffered from mental health issues; domestic violence; family breakdown; teenage pregnancy; managing children’s behaviour; poverty; refugee and asylum issues; children with learning difficulties.

  • Between two to four people sharing a beautiful house.
  • Each young person has their own room decorated to their taste.
  • Person-focused programmes of support.
  • TV plus SKY.
  • Free unlimited Wi-Fi.
  • Round-the-clock staff support.
  • Independent living skills are guaranteed by the end of the programme.
  • Employment and further education opportunities enhanced.
  • We welcome, and are trained to support people with a learning disability, as well as Unaccompanied Asylum Seeking Children (UASCs).
